CTHCS is a newly formed multi-purpose service which provides a flexible array of services to meet the needs of people residing in this rural catchment area.The Community Services division provides six programs. Community Options Program - case or care management for frail aged, younger people with disabilities and their carers. For those with high level and/or complex needs. Focus is to keep people in their own homes and community for as long as possible. Services may include home maintenance, gardening, personal care, meal service. Community Care - social support for frail aged, younger people with disabilities and their cares. Activities include centre based activities, monthly bus trips, men's day trips monthly, information and advocacy, referral to other agencies if required, equipment, assistance with community business, home visits. Transport - for people who have no other means of transport and are unable to access public transport. Home Care - assists in providing a range of basic support services for frail aged, people with disabilities and their carers. Services may include cleaning, vacuuming, assistance with meal preparation or making the bed. Respite Options - designed to provide support, information and well earned 'time out' for carers of people who suffer from dementia or challenging behaviour. Volunteers - with the assistance of volunteers the services would struggle.
