Community Options Service (South)

Community Options is a community care case management service. Clients are assisted with accessing, organising and co-ordinating community care services, with the aim to ensure that the appropriate services are provided to enable clients to remain living in there own homes. To become a client of Community Options, a person must be a frail aged person or a carer or a younger person with a disability. Further, clients must be assessed as having high and complex care needs that require a co-ordinated approach to service delivery. Community Options has a Service Manager and 3 Case Managers and a part time administrator, who are able to assist 60 clients in the Hobart Municipality.
